When you come for treatment at Pelvic and Core Physiotherapy, information is gathered that allows the therapist to make an informed professional decision as to the most appropriate assessment and treatment methods to be used. This information will include basic demographic data (e.g. name, date of birth, address, etc), as well as other information that is considered relevant. This will usually include the following:

  • The nature of the problem for which you are presenting;
  • A medical history;
  • Other information deemed necessary to make an informed clinical judgement about the nature of the presenting complaint and other relevant factors

Security:

Information provided to Pelvic and Core Physiotherapy is held both in the Client File and the Clinic Database. All files are held within the Clinic in locked filing cabinets or held electronically in a secure manner.

I am committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, I have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information I collect online.

All your electronic records are held safely and securely on password protected devices, or within systems hosted by third party providers, also known as data processors, who are legally and contractually bound to operate with security arrangements that are in place to protect your data safely and securely.

Should I require the services of any other third party suppliers, I will ensure they are also legally and contractually bound to protect your information.

Cookies

How long your electronic data is retained

If you leave a comment, the comment and its metadata are retained indefinitely. This is so I can recognise and approve any follow-up comments automatically instead of holding them in a moderation queue.

  • I may ask permission to contact your medical team, such as your consultant, GP or any other therapist who is treating you to ensure there are no contra-indications to treatment.
  • Practitioners at Pelvic and Core Physiotherapy may discuss a client, with your consent (though not full identifying information) within the pelvic health network if they have any concerns or to ensure treatments are being adapted in the best way for that individual to meet their goals.
  • You may request not to be discussed within the network.
  • In the event that you give permission to share information, images or video testimonials by completing and signing a written consent form.

What rights you have over your data

Requests should be specific as to the exact information required, and should be acknowledged within 14 days in writing. Once a request for information is received, Caroline Callister will conduct a thorough database review and collate the information in an appropriate manner to ensure adequate and full understanding on the part of the requesting person.
